General Terms and Conditions:
- Your card has been pre-loaded with $500 in savings and ed and may be redeemed at Charles P. Virden, M.D., F.A.C.S. or Renovation, "Your Ultimate Medical Spa."
- For service at Renovation, "Your Ultimate Medical Spa:" Present your card before your purchase or service and we will deduct 15% off the total retail value.
- For procedures, except insurance related, at Charkes, P. Virden, M.D., F.A.C.S.: Present your card after you have scheduled a procedure to receive 15% off the total due. Card does not apply toward insurance related cases, revisional procedures, operating room costs or anesthesia.
- All discounts will be deducted from the balance of your card. You may use your card as often as you like until your balance is $0 or until your card expires on May 31, 2010.
- One card per transaction
- Your card is transferable to family & friends
- Your card may not be valid with other offers or advertised specials
